In this dynamic role, you will work directly with vulnerable populations, providing essential support and guidance as they navigate their paths towards stable housing and improved well-being. You will be part of a multidisciplinary team, collaborating with social workers, mental health professionals, and community organizations to deliver comprehensive services that address the diverse needs of our clients. Your role will involve developing individualized support plans, conducting outreach, and facilitating access to vital resources such as social services, employment training programs, and mental health services. Responsibilities
Provide direct support to individuals experiencing homelessness, offering guidance and practical assistance in accessing housing and related services.
Conduct assessments to identify the specific needs and challenges faced by clients and develop individualized support plans.
Collaborate with a multidisciplinary team to coordinate services and resources for clients, including mental health support, job training, and healthcare access.
Engage in outreach efforts to connect with individuals in need, building trust and rapport to facilitate their engagement with support services.
Maintain accurate and up-to-date records of client interactions, progress, and outcomes to ensure effective service delivery and reporting requirements.
Advocate on behalf of clients with external service providers, ensuring they receive the necessary support and resources to achieve stability and independence.
Participate in regular team meetings, training sessions, and professional development opportunities to continually enhance skills and knowledge in homelessness and support services.
Requirements
Proven experience working with individuals experiencing homelessness or in similar social services roles.
Strong understanding of the complex factors contributing to homelessness, including economic, social, and psychological elements.
Excellent communication and interpersonal skills, with the ability to build rapport and trust with clients from diverse backgrounds.
Demonstrated ability to work independently and as part of a team in a fast-paced and challenging environment.
Strong organizational skills and attention to detail, with experience in case management and record-keeping practices.
A genuine commitment to advocating for social justice and the well-being of marginalized individuals.
Relevant qualifications in social work, psychology, or a related field preferred; training in trauma-informed care is a plus.
