We are currently seeking a highly skilled and motivated individual to support the team to manage a complex caseload of patients and provide leadership for the administrative element of the team. Alongside these duties, as part of the senior team, you will be expected to support the junior members of the multidisciplinary team and support the clinical services manager in day-to-day management and service development.The postholder will ensure that all service procedures and processes are effective and safe and enable the service to meet its contract targets and deliver a high-quality patient experience. You must be flexible, organised and have excellent time management skills as the team priorities change frequently throughout the day.

You will be required to work weekends on a rota basis as required to ensure cover for the service 7 days a week.



Undertake the comprehensive assessment of patients, including those with complex presentation, multi-pathology, using investigative and analytical skills and to formulate individualised management and treatment plans.

Excellent clinical reasoning will be demonstrated and utilising a wide range of treatment skills and options to formulate a specialised programme of care and to enable individuals to remain in their own home.

Demonstrate the use of advanced clinical knowledge to identify changing or deteriorating health conditions.

Use independent non-medical prescribing skills and knowledge to facilitate treatment in a professional and timely manner for conditions within the scope of individual’s competences.
