As a Band 6 Team Sister / Charge Nurse, you will: Support the Team Lead with the day-to-day coordination of the community nursing service Provide clinical leadership and act as a role model to the wider nursing team Undertake holistic assessments and deliver evidence-based nursing care within patients homes and community settings Coordinate care plans and ensure continuity of care for patients with complex health needs Promote patient independence and support rehabilitation and recovery goals Work closely with the multidisciplinary team including GPs, therapists and social care colleagues Ensure a safe and effective service through appropriate clinical governance and risk management Maintain accurate and contemporaneous patient records Communicate effectively with patients, carers and the wider healthcare team Support junior staff, students and colleagues within the team Act in accordance with the NMC Code of Conduct
