Crynodeb
The Care Navigator will support the clinical team by signposting patients to the most appropriate healthcare professional or service, ensuring patients receive the right care at the right time. Working as part of the practices multidisciplinary team, the Care Navigator plays a key role in improving access to services and enhancing patient experience. The role will also include reception and administrative duties, supporting the wider team in delivering a polite, professional and efficient service to the practices patient population.
