Supports health promotion and prevention activities across the PCN, including smoking cessation, weight management, and lifestyle interventions Plays a key role in vaccination and immunisation programmes within the PCN Works within a Multi-Disciplinary Team to support patients with long-term conditions and health inequalities Provides clinical support to registered nurses and other healthcare professionals within the PCN Delivers basic clinical care interventions under supervision, supporting continuity of care and patient safety initiatives Taking and recording blood pressure, height & weight Performing ECGs Performing phlebotomy in accordance with local and national guidelines Correctly labelling, handling, storing and transporting blood samples in line with pathology requirements Assisting with chronic disease monitoring (e.g., diabetes, hypertension, asthma, COPD) Conducting NHS health checks and cardiovascular risk assessments Assisting GPs and nurses with minor procedures Updating patient records and ensuring accurate documentation using clinical systems (e.g., EMIS / SystmOne) Maintaining infection prevention and control standards at all times UK Phlebotomy Skills & Qualifications: Competent in phlebotomy for adults Knowledge of correct order of draw, appropriate sample bottles, and pathology test requirements Understanding of specimen labelling, cold chain requirements, and safe transportation procedures Awareness of potential complications (e.g., fainting, haematoma, failed venepuncture) and appropriate escalation procedures Knowledge of infection prevention and control, including hand hygiene and use of PPE Understanding of health and safety legislation including sharps management and needle-stick injury procedures Ability to recognise and escalate abnormal results or clinical concerns appropriately Completion of an accredited UK Phlebotomy Training Course or equivalent competency-based training Evidence of supervised practice and signed-off competency in venepuncture Up-to-date Basic Life Support (BLS) and anaphylaxis training Personal / Professional Development: Post holder must hold a Care Certificate or work towards one Evidence of ongoing CPD relevant to clinical practice and phlebotomy Participation in an annual individual performance review, including maintaining a record of own personal and/or professional development Taking responsibility for own development, learning and performance and demonstrating skills and activities to others undertaking similar work Inform the Lead GP or line manager of any concerns regarding role and professional development needs Be aware of own professional boundaries and what to do when these are reached Quality: The post-holder will strive to maintain quality within the Practice, and will: Alert other team members to issues of quality and risk Work to practice protocols, national guidelines, and CQC standards Assess own performance and take accountability for own actions, either directly or under supervision Contribute to team effectiveness by reflecting on activities and suggesting improvements Work effectively with individuals in other agencies to meet patients needs Effectively manage own time, workload and resources Contribution to the Implementation of Services: The post-holder will: Apply Practice policies, standards and guidance Discuss with other team members how policies, standards and guidelines affect own work Participate in clinical audit where appropriate Support service development initiatives within the PCN
