Job Summary To provide accessible, early intervention wellbeing support for children and young people (CYP) and their families/carers through brief, evidence based interventions delivered on site or remotely. As part of this role, you will be expected to collect, analyse and report outcomes data, ensuring high data quality, robust governance, and meaningful feedback that demonstrates value and drives continuous service improvement and meets requirements set out by PCN member practices. You will work across the 3 member practice sites with a weekly dedicated day to each site. Key Responsibilities Key responsibilities will include but are not limited to: Complete initial assessments to identify, triage and respond to risk, following local safeguarding children & escalation procedures as required Provide early intervention wellbeing support where appropriate to support mild-moderate emotional wellbeing needs via 1:1 or group sessions Involve parents/guardians where appropriate and provide brief parental support aligned to the CYPs goals Coordinate with GPs, CAMHS, schools, and VCFSE partners; make timely referrals and contribute to multiagency working where appropriate Record clinical outcomes and report on KPIs to demonstrate service performance Uphold data governance (IG/GDPR), collect consent, and maintain confidentiality Demonstrate motivation, reliability, flexibility, and commitment to the role Establish and maintain good working relationships with the GP practice teams Meetings It may be necessary to attend and contribute to various Practice, PCN and/or DHC meetings and forums To attend regular meetings with your line manager, feeding back updates, issues or challenges Participate in educational events/meetings Confidentiality The post holder must maintain the confidentiality of information about patients, staff, PCN and DHC business in accordance with the Data Protection Act 1998 and Caldicott principles. Health and Safety To have an awareness of national standards of infection control and cleanliness and regulatory, contractual, professional requirements, and good practice guidelines Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients Monitor work areas and practices to ensure they are safe and free from hazards and conform to the practices health & safety and infection control policies To engage in audits as required and appropriate To log all faults and arrange replacements/repair where necessary and appropriate To report any risks and undertake Significant Event Analysis where appropriate Training and Personal Development Personal development will be encouraged and supported by DHC and the PCN It is the individuals responsibility to remain up to date with relevant developments within the service Statutory and mandatory training is undertaken annually through e-learning You are responsible for holding and updating your Personal Development Plan which will be reviewed annually at appraisal Participate in the education and training provided by the Federation/PCN Make effective use of learning opportunities within and outside the workplace, evaluating their effectiveness and feeding back relevant information If it is necessary to expand the role to include additional responsibilities, full training will be given
