Crynodeb
The post holder will support the management of transitional care, young adults and adults working alongside a highly skill team of consultants, specialist nurses in paediatrics , adults and specialist dieticians. In additional to your clinical role, you will aim to raise the awareness and improve standards of diabetes care through planning and implementation, of specialised programmes ofeducation and training within your area of specialty. You will Act as an expert educational resource and support to all grades of staff in intermediateservice, GP Practices and Community Nurses in the delivery of comprehensive diabetes carefor transitional patients and young adults. You will manage your own caseload and participate in and, where appropriate, initiate research projects.
