Post: Brokerage Assistant
37 Hour Post 9am 5pm (ad hoc bank holiday cover)
Team: Service Brokerage Health and Care Department, Community Commissioning Division
Main Base: Textile Hall, Market Street, Bury (the team may move to 6 Knowsley Place or Bury Town Hall in the future)

About our team and the expectations of your role
Brokerage is a small team that delivers significant impact across the social care system. The team is highly responsive, working with customers as well as assessment and social care teams to ensure needs are met in a timely and effective way.
We will provide a working environment that is supportive and rewarding, while working as part of a team who are passionate about helping people and the work they do.
You will be supported by two Brokerage Assistants, a Brokerage Supervisor, and the Brokerage Manager, all of who will help you develop skills and confidence as you grow into the role.
The main function of the Brokerage team is to support access to the Right Care, at the Right Time, in the Right Place for all people who are referred to our service.
You’ll play a key role in reviewing assessments referred to us ensuring strengthsbased practice is at the heart of our work, before coordinating with commissioned care providers to put the right support in place within required timescales.
You will support Home First principles, providing uptodate information on capacity and demand, and offering advice and guidance on the market to support informed decisionmaking across departments.
You will support with individuals across homebased and bedbased services, coordinating hospital discharge pathways and working closely with Integrated Neighbourhood Teams to ensure that people receive the appropriate care.
You’ll be adaptable, with a positive, “let’sdo” attitude, working as part of a team that delivers for our customers. Your actions will reflect your commitment to both the team and the organisation.
About you
Brokerage is a bespoke service within Bury, operating with its own unique processes and a structured induction is in place to ensure staff are wellequipped to deliver the service effectively.
You will be;
- Person centred and compassionate, you'll possess excellent communication skills, be highly organised, and be a team player.
- Passionate about helping people to meet their care and support needs in a strengths- based way in order to live as independently as possible.
- Able to build and develop trusting professional and responsive relationships with a variety of stakeholders, from people and families who require services, care providers, social workers, and other teams across the footprint of Bury.
- Aware of the legal frameworks relevant to Adult Social Care including the Care Act, Deprivation of Liberties in care homes, and Safeguarding Adults.
- Able to use Information technology systems and learn new processes and skills quickly.
- Able to support and advise teams and individuals to aid professional discussions.
- Able to demonstrate excellent written and verbal skills
- A reliable and supportive member of the team, uphold our values, and be keen to develop.
- Able to evidence a clear DBS for Adults
