Job Responsibilities Service Flexibility As the organisation operates under an annual service contract, the duties and focus of this role may change in line with service requirements. The post holder will therefore be expected to demonstrate flexibility in supporting service delivery, as responsibilities may vary from year to year. Clinical Nursing Practice The Advanced Nurse Practitioner (ANP) will deliver high-quality, patient-centred care within the practice by: Working collaboratively within a multi-disciplinary team to support the health and wellbeing of a diverse patient population. Acting as the first point of contact for patients with undifferentiated conditions; undertaking clinical assessment, diagnosis and developing management plans. Managing acute and chronic conditions using evidence-based pharmacological and non-pharmacological treatments tailored to individual needs. Providing holistic care focused on prevention, self-care and health promotion for patients and families. Prescribing and reviewing medications within scope of practice, advising on side effects and interactions. Coordinating patient care, involving patients and carers in decisions and supporting adherence to treatment plans. Requesting and interpreting diagnostic tests within agreed protocols to inform clinical decision-making. Prioritising and triaging patient needs, managing urgent and emergency situations effectively. Undertaking mental health assessment and making referrals in line with national guidelines. Delivering care within both practice and community settings and making appropriate referrals where required. Maintaining accurate, contemporaneous clinical records in accordance with NMC and practice standards. Supporting practice contractual requirements and contributing to performance monitoring such as QOF and other key performance indicators. Supporting and mentoring nursing staff and promoting evidence-based practice. Care Management Communicating sensitive information effectively to patients, relatives and carers. Providing specialist advice and making referrals, including urgent or fast-track referrals where appropriate. Contributing to local and national clinical targets and monitoring patient outcomes. Health Improvement Managing health screening programmes and promoting health priorities within the practice team. Supporting innovations and initiatives aimed at improving patient outcomes and disease prevention. Making appropriate referrals to statutory and voluntary agencies. Administration & Professional Responsibilities Producing accurate and complete patient records and ensuring clinical systems remain up to date. Ensuring appropriate completion of documentation and claims related to patient care. Participating in practice meetings and maintaining safe and effective clinical environments. Training & Personal Development Maintaining advanced clinical skills and professional knowledge through continuous learning. Participating in appraisal, peer review, clinical supervision and reflective practice. Contributing to staff education and supporting student training programmes. Developing and maintaining a Personal Learning Plan in line with revalidation requirements. Confidentiality Complying with data protection legislation and maintaining strict confidentiality of patient, staff and practice information. Health & Safety Identifying, reporting and managing health and safety and infection control risks. Following national standards relating to infection control, sharps management, waste disposal and spill management. Promoting safeguarding and the welfare of children and vulnerable adults. Maintaining cleanliness and safety standards within clinical and general areas. Equality & Diversity Respecting and promoting the rights, dignity and beliefs of patients, carers and colleagues. Communicating sensitively during difficult conversations, including explaining diagnoses and care plans. Contribution to Service Development Applying and contributing to practice policies, standards and clinical guidelines. Participating in clinical audit and quality improvement initiatives. 37.5 hours per week, scheduled between 8:00am and 6:30pm, Monday to Friday, in line with service requirements. Operational hours may be subject to change in line with organisational or contractual requirements.
