Coordinate and support the delivery of group consultations (group clinics) for patients with low-level mental health needs and MSK conditions. Identify suitable patients using practice searches and population health data, and support invitation processes. Manage bookings, appointments, reminders, attendance records and practical arrangements (venues, IT setup, materials). Support the preparation and follow-up of group sessions, ensuring patients receive appropriate information before and after attendance. Work with GPs and the multidisciplinary team to identify, monitor and manage a caseload of patients who would benefit from personalised care coordination. Support patients to develop, review and update personalised care plans, ensuring goals are clearly recorded and agreed. Provide care navigation, signposting and coordination across health, community and voluntary sector services. Work alongside the Work and Health Coach to support patients on fit notes, including preparation for work-related discussions and appropriate referrals. Collect and record outcome measures, attendance data and patient feedback to support service evaluation and improvement. General administrative responsibilities: Maintain accurate, timely and confidential records on clinical systems in line with information governance requirements. Manage correspondence, emails and telephone queries relating to group consultations and care coordination. Support scheduling of meetings, MDT discussions and service activities where required. Prepare documents, reports and data extracts to support monitoring and reporting. Use Microsoft Office and clinical systems effectively (training provided where necessary). Contribute to audits, service reviews and quality improvement activities. Attend relevant training, supervision and team meetings. Work in accordance with safeguarding policies, escalating concerns appropriately.
