Crynodeb
Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology. Regularly report and effectively communicates service-users health status and care aims in care records and verbal reports to allocated workers and other members of the multi-disciplinary team Ensure clinical records are of a high standard and updated as required. At all times ensures that own actions, support the equality, diversity, rights and responsibilities of individuals At all times minimizes risks to self by undertaking safe working practices whilst regularly working under remote supervision in community settings. At all times works within agreed and established Trust policy and operational procedures. Works with other Trust services to promote seamless service-user focused care, transfer and discharge on identified care pathways including accepting routine referrals during the day. Responsible for contribution towards planning and undertaking delivery of planned physical health care observations. Regularly provides advice and guidance promoting healthy activities and lifestyle choices. At all times takes responsibility for own personal and professional development and supports the development of colleagues, students, trainees and volunteers.
