Salary: £28,605 - £31,029 per annum (pro rata for part-time) or £14.82 - £16.07 per hour

About the Role

The HART service is to provides several care & Support offers under its registration as a domiciliary care service. This includes, reablement, which helps people return home from hospital, or prevent them from being admitted to hospital, and regain their independence. Homecare support to facilitate the timely discharge from acute and community hospitals to better understand the needs of the person once they have been able to return home. Short term crisis support, for those experiencing an urgent social care need. These service offers are all short-term support, helping someone to become as independent as is possible, and to ensure that the right support pathway is in place to meet any ongoing care and support needs

Special Factors

The nature of the work may involve the jobholder carrying out work outside of normal working hours. Ability and willingness to travel and work from various locations throughout the County and travel between sites as required

  • This post is eligible for a DBS check under the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (i.e. it involves certain activities in relation to children and/or adults) and is defined as regulated activity under Part 1 of the Safeguarding Vulnerable Groups Act 2006. Therefore, a DBS enhanced check for a regulated activity (includes a barred list check) is an essential requirement
  • Ability to work flexible hours on a rota basis between 10.30pm and 7.30am 365 days a year rota, including weekends, evenings and Bank Holidays.

About You:

  • NVQ level 2 in a relevant field or other equivalent qualification in a relevant subject or equivalent level of relevant experience working with adults in a social care setting.
  • Experience of working with other agencies and/or working in a multi-agency environment to identify and meet need with creative and pragmatic solutions.
  • Knowledge of work with people from all walks of life, and the desire to provide good quality services within allocated resources.
  • Ability to assist with moving and handling of people who use the service.
  • Ability to communicate clearly and accurately, both orally and in writing, with a wide range of people, including representatives from other agencies, using skills of negotiation and persuasion
  • The ability to prioritise effectively across a wide range of competing demands to meet deadlines.
  • An understanding of the Council’s Adult Social Care strategies, relevant regulations/ legislation and service frameworks underpinning practice in community care and residential settings. You must have a full UK driving license and access to a vehicle for work.
