Crynodeb
To maintain and build on effective partnership working arrangements within the city, including Neighbourhood teams, Community Care Collaborative staff, Commissioners and external companies, to support and develop integrated service delivery. To advise, guide and engage in care planning relevant to nutrition nursing. To make a significant overall contribution to service development, ensuring that the service is up to date and fit for purpose, writing and developing processes, standards and policies as necessary. To ensure that clinical objectives are set for the pathways, which incorporate the Divisional and Trust objectives, maximise opportunities for integrated working and consider local needs, issues and access. To be responsible for standard setting, guidelines and policies, to ensure that they are current and adhere to a high standard, including implementation of NICE guidelines. Full duties please see JD.
