Clinical Practice To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to GPs, and other members of the practice and primary care team Assess, plan, develop, implement and evaluate treatment plans that promote health and wellbeing, and for patients with long term conditions Proactively identify, help in diagnosing and manage treatment plans for patients at risk of developing a long-term condition Undertake long-term condition management clinics, such as COPD, Spirometry, Asthma, Diabetes, and cardiovascular disease Initiate and deliver health promotion and screening programmes, including cervical screening and sexual health screening Assess medications for therapeutic effectiveness, ensuring alignment with patient needs and evidence-based guidelines Work with patients in order to support adherence to prescribing treatments Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Support patients to adopt health promotion strategies that promote patients to live healthy, and apply principles of self-care Support and manage health needs of women presenting for family planning and cervical cytology consultations Recognise, assess and refer patients presenting with mental health needs in accordance with the NSF for Mental Health. To be aware of local mental health care pathways, and referral/support systems Implement and participate in vaccination and immunisation programmes for both adults and children Advise on, support and administer vaccinations where appropriate for patients travelling abroad Provide a holistic approach to wound management and implement wound care in line with current evidence-based guidelines including the use of doppler Where the post holder has completed an approved prescribing qualification for the role: to prescribe safe, effective and appropriate medication as defined by current legislative framework and within personal competence and agreed formulary boundaries To actively participate in the delivery of existing targets for General Practice relating to patient care (QOF), prescribing and others as deemed by the arrangements in the general practice contract Communication Apply sensitive communication styles to ensure patients are fully informed and consent to treatment Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating Establish and maintain effective communication with individuals and groups within the practice environment and with stakeholders Advocate for patients when representing them Quality Service Delivery Work within the boundaries of personal competence and uphold professional standards as set out by the NMC Maintain accurate, timely, and comprehensive records of all patient consultations in line with relevant legislation, policies, and procedures Prioritise, organise, and manage workload effectively to ensure highquality care is consistently delivered Evaluate the effectiveness of care through selfreflection, peer review, benchmarking, and structured evaluation processes Contribute to the development and maintenance of quality governance systems and processes within the practice Take part in team performance reviews and provide constructive feedback where appropriate Understand and apply legislation and policy relating to the safeguarding of vulnerable or abused children and adults, including awareness of statutory procedures and local guidance Adhere to policies relating to domestic abuse, safeguarding of vulnerable adults, substance misuse, and addictive behaviours, ensuring appropriate referrals are made Reflect on personal performance and take responsibility for actions, whether working independently or under supervision Collaborate effectively with professionals from other agencies to ensure patients needs are met Personal Development Take responsibility for personal learning, development, and performance, actively engaging in clinical supervision and serving as a positive role model by demonstrating skills and practices to colleagues undertaking similar roles Contribute to and participate in the creation, review, and implementation of local guidelines, protocols, and standards Engage fully in any training programmes provided by the practice as part of the role Take part in an annual performance review and maintain accurate records of personal and professional development activities Team Working Understand the scope and responsibilities of the role and recognise how it may evolve over time Contribute as an effective, accountable, and supportive member of the team Engage in team activities that promote opportunities to enhance patient care Collaborate with colleagues to establish shared values, direction, and policies that influence the delivery of care Risk Management Manage and assess risks within areas of responsibility, ensuring appropriate measures are in place to safeguard both staff and patients Monitor the work environment and practice setting to ensure they remain safe, hazardfree, and compliant with all relevant health, safety, and security legislation, policies, and guidelines Ensure the safe storage, rotation, and disposal of vaccines and medications Implement and adhere to infection prevention and control measures within the practice Full job description attached to advertisement for further details.
