JOB PURPOSE This role provides specialist dementia expertise within Ashfield North PCN. It is open to Registered Nurses with dementia specialism The post holder will: Deliver specialist, relationship centered dementia care. Support people with dementia and their families across the full dementia pathway from early diagnosis to end of life care. Provide expert assessment, complex case management, and evidence based interventions. Offer clinical leadership, education, and support to PCN staff and system partners. Contribute to service development, pathways, and dementia strategy work. To see patients and or families in GP Surgeries, patients homes, or other locations. KEY RESPONSIBILITIES 1.Clinical Duties (applies to both professional backgrounds) Provide advanced dementia specific assessment using appropriate professional frameworks e.g. Admiral Nurse Framework Manage complex dementia presentations, including distressed behaviours, carer strain, physical and mental health deterioration, and safeguarding concerns. Develop personalised care plans, including risk management, behaviour support strategies, and wellbeing-focused interventions. Act as a key worker for families with complex needs, coordinating care across primary care, mental health, social care, voluntary sector and urgent care. Support advance care planning, best interest decisions, and end of life conversations. Attend and contribute to MDT meetings and case discussions. Make appropriate referrals within professional scope. Provide clinical advice to GPs ANPs social prescribers and other PCN roles. 2. Profession-Specific Duties Undertake specialist nursing assessments including physical health monitoring and interpretation linked to dementia care. Provide clinical leadership on medication-related issues, recognising red flags requiring GP or specialist intervention. Maintain NMC standards and use the Admiral Nurse Competency Framework where aligned. Any other relevant Nurse duties related to QOF or similar 3. Education, Training and Leadership Deliver dementia training to PCN teams, GP trainees, social prescribers, care home staff and community partners. Act as a role model for dementia inclusive person-centred practice. Support the implementation of PCN dementia pathways quality projects and audits. 4. Evaluation and Audit Collect and analyse service data to support evaluation and improvement. Contribute to dementia strategy groups and provide written reports whererequired. Participate in audit cycles and research activities relevant to dementia care. 5. Professional Requirements Maintain professional registration NMC. Engage in CPD dementia specific training and supervision. Adhere to PCN and organisational governance, safeguarding, and information governance policies.
