Crynodeb
The post holder will be responsible for assessing and managing patients through face-to-face consultations, telephone assessments, and home visits. They will undertake comprehensive clinical assessments, develop and implement appropriate management plans, and provide treatment, referrals, and safety-netting advice as required. The role involves managing patients with undifferentiated and complex presentations, recognising potential clinical risks, and escalating cases to a GP or other appropriate services when necessary. The Paramedic Practitioner will request and interpret appropriate investigations within their scope of practice and support patients by providing health promotion, self-care advice, and guidance to help them manage their conditions effectively. The post holder will work under appropriate clinical supervision from a GP and will seek guidance where required to support safe and effective decision-making. They will also support the duty doctor by assisting with patient assessments, managing appropriate same-day demand, and contributing to the safe triage and management of urgent clinical presentations. Working collaboratively with GPs and the wider multidisciplinary team, the post holder will contribute to clinical discussions and ensure continuity of care. They will maintain accurate, timely, and comprehensive clinical records in accordance with professional standards and clinical governance requirements.
