Crynodeb
Deliver evidence-based nursing care to patients in line with NHS standards. Assess, plan, implement, and evaluate individualised care for patients. Administer medications and treatments following protocols and guidelines. Perform clinical procedures safely and efficiently, ensuring patient comfort. Maintain accurate and timely documentation of patient care using the appropriate clinical system with a focus on data completeness and audit. Ensure adherence to infection control policies and safeguarding guidelines. Actively participate in multidisciplinary team meetings and patient case discussions. Provide health education and emotional support to patients and their families. Participate in clinical audits and service improvement projects. Contribute to research and innovation in nursing practice. Provide nurse led clinics. Undertake comprehensive lung screening health risk assessments. Assess patients suitability to proceed to a diagnostic test, explain and take consent for the diagnostic test, and submit the diagnostic test referral form. Review and triage the LDCT reports and triage appropriately supporting setting up of the Screening Review Meeting (SRM) for those patients with significant findings Uses specialist knowledge and skills to provide practical help and emotional and psychological support to both patients and their families, and practical help through effective communication around CT scan findings Provision of joint service cover within the lung cancer nursing team as required, for the benefit of patient care and service standards if adequate staffing within the Lung Cancer Screening team allows Possess a working knowledge of respiratory equipment. For example: inhalers, spirometers and compressors Work collaboratively with multidisciplinary teams to ensure seamless referral pathways and continuity of care for individuals identified as needing further interventions Ensure patient focus remains at the forefront of all decision making in liaison with the Lung Cancer Screening Responsible Accessor and the wider SRM (Screening Review Meeting) and MDT (Multidisciplinary Team) Ability to provide health promotion and lifestyle advice with referral to stop smoking service Be a key person in conjunction with the Lung Cancer Screening Responsible Accessor in setting up and running of the new service delivery model through assessment of pathways, planning and evaluation
