The Opportunity
We have a fantastic opportunity to join a leading healthcare provider as a patient care coordinator. You’ll be supporting patients who rely on homecare services. You’ll play a vital role in coordinating treatment deliveries, managing patient communication, and ensuring every individual receives a seamless, compassionate service.

Your duties and responsibilities will be

- Making timely outbound calls to patients to arrange their scheduled support and processing their treatment requirements accurately.
- Working closely with clinical teams to ensure each patient’s ongoing care needs are fully monitored and maintained.
- Managing planned call schedules and daily order processing to ensure treatments are dispatched correctly and on time.
- Tracking patient stock levels to avoid shortages or excess supply while staying aligned with internal stock policies.
- Organising urgent or out-of-cycle deliveries, rearranging appointments when needed, and keeping distribution teams updated.

You will have the following qualifications & experience:

- Previous experience in a customer-facing role, whether via telephone or in person.
- Strong administrative abilities, with accuracy when handling patient records, new registrations, and closures.
- Confident communication skills and the ability to manage sensitive or challenging situations professionally.
- The capacity to stay calm and efficient under pressure in a busy environment, while showing empathy to patients and carers.
- GCSEs in Maths and English (grade C or above) or an NVQ in Customer Service, plus the ability to work collaboratively across departments.

It’s great if you also have the following

- Experience working within a healthcare, pharmacy, medical devices, or homecare service environment.
- Familiarity with patient management systems or CRM platforms (e.g., Ulysses, EMIS, CareFlow, or similar).
- Understanding of prescription processes or knowledge of medication supply chains.
