This role delivers a nursing service that provides high quality care to patients and meets clinical and regulatory requirements. You will assess plan and deliver high quality patient care and support patients and carers to understand their condition medication and lifestyle needs so they can manage their own health with confidence. You will work with internal and external colleagues and ensure all care is safe effective and compliant. Key accountabilities Assess plan deliver and evaluate high quality care for a caseload of patients with varying conditions and needs Carry out comprehensive assessments and create individualised care plans updating these as the patient condition changes Administer prescribed medications treatments and therapies Monitor vital signs and conduct ongoing clinical assessment using sound clinical judgement and risk based decision making Identify and escalate changes in condition and manage sudden deterioration or emergencies safely Work respectfully with patients families and colleagues recognising diverse cultural religious and ethical needs Work with other health professionals to provide holistic evidence based nursing care Recognise limits of competence and seek advice where needed Provide on call support where required Education and support for patients and carers Provide clear information on disease management medication management lifestyle changes and early warning signs Answer questions to support understanding and confidence in managing care at home Identify environmental risks and advise on safety Act as an advocate offering support guidance and signposting Provide emotional support to patients and families Quality safety and compliance Follow all clinical governance requirements policies procedures and professional standards Document all care clearly and promptly in line with regulatory requirements Report incidents near misses adverse events and safeguarding concerns Take part in audits risk assessments and service reviews and contribute to improvements Maintain confidentiality and protect patient information Communication and collaboration Communicate effectively with patients families and the wider multidisciplinary team Explain clinical information in clear and accessible language Work with nursing service managers and colleagues to identify risks opportunities and improvements Enablers for the role Active registered nurse with current NMC registration Clinical experience in relevant specialties Competence in clinical procedures such as IV administration medication management and patient assessment Ability to make independent decisions and manage unexpected challenges Strong verbal and written communication skills Ability to build rapport and work respectfully with patients and families Understanding of safety procedures in home or community settings Accurate and timely documentation Adaptability to different environments and changing schedules Ability to teach patients and families about medications care tasks and condition management Full valid UK driving licence
