Crynodeb
Support the completion of screening tools for CYP and their families and schools, referred to the team, to identify the CYPs strengths and needs. To complete a well-being assessment, either virtually or face to face, with the CYP and their family, to review the CYPs identified strengths and needs and discuss reasonable adjustments to support meeting those needs. To signpost the CYP and family to local support offers and web-based information that may be beneficial in meeting the needs of the CYP and family and, where feasible, help the family to access these support opportunities. To network with health, education and social care colleagues and networks to support the development of a service directory of support for each borough. To participate in clinical triage and multi-disciplinary team case discussions, providing input following the wellbeing assessments. Supported by the senior clinicians and the ND Wellbeing Practitioner, monitor CYP who meet the diagnostic criteria for ADHD/ASD, providing ongoing support and interventions where identified and appropriate to do so within the limits of competence. To support the ND Wellbeing Practitioner with the delivery of the patient education workshops, including identification of topics, alignment to the most appropriate clinician, patient selection and facilitation of the sessions.
