Please refer to the attached job description and person specification for full details of responsibilities Mental Health practitioners will provide assessment and interventions for people with significant mental health conditions such as dementias, depression, psychotic illness and personality disorders. Offer training, advice and guidance to colleagues on common mental health conditions. Participate in MDT meetings and ensure key information is recorded in shared care plans Work closely with the mental health integrated care team in order to provide an excellent mental health service. Clinical Assessment of mental health needs. Assessment and management of risk. Participating in a duty rota to offer on call support and advice to service users and their carers. Holistic care planning and co-producing crisis plans with service users, keeping service users and their families at the heart of the intervention.
