To be responsible for the assessment, planning, implementation and evaluation of person-centred care for adults with primary mental health diagnoses who also present with complex physical health needs, comorbidities, and increasing frailty. The post holder will provide clinical leadership, act as a named nurse, and support the Registered Manager in the safe, effective, and compliant operation of the nursing home, deputising in their absence. The role ensures the service meets its Statement of Purpose, complies with CQC regulations, NMC standards, Mental Capacity Act, safeguarding legislation, and internal governance frameworks, while promoting recovery, rehabilitation, dignity and social inclusion. Person-Centred Mental Health & Psychiatric Care Act as a named nurse, ensuring person-centred planning is central to care delivery. Demonstrate comprehensive knowledge of psychiatric conditions, including: Psychotic disorders Mood disorders Personality disorders Cognitive impairment and dual diagnosis Assess, monitor and respond to changes in mental state, emotional wellbeing, and relapse indicators. Lead on the management of displayed behaviours, ensuring: Behaviour Support Plans are in place, reviewed, and followed. ABC charts are analysed, patterns identified, and actions implemented. Interventions are least restrictive, trauma-informed, and proportionate. Ensure behavioural incidents are managed safely, documented accurately, reviewed and used to reduce recurrence. Ensure Regular reviews take place with CPNs and psychiatrists. Physical Health, Frailty & Complex Nursing Care Deliver and oversee physical health and complex nursing care for residents with mental health needs and increasing frailty, ensuring care is safe, proportionate, and person-centred. Provide and manage nursing interventions including (but not limited to): Catheter care o PEG feeding and management Stoma care Syringe driver use where clinically indicated. Monitor physical health through clinical observations, early warning signs, and escalation of deterioration in line with policy and professional judgement. Provide wound care, dressing changes, and wound assessments, monitoring skin integrity and preventing avoidable harm. Review and monitor nutrition, hydration, and weight, taking appropriate action to reduce risk and promote wellbeing. Ensure positional changes and pressure care interventions are completed as scheduled to prevent pressure damage and promote comfort. Manage long-term physical health conditions, including diabetes care and blood glucose monitoring. Provide end-of-life and palliative nursing care where required, ensuring care is compassionate, dignified, and aligned with advance care planning, Mental Capacity Act principles, and multidisciplinary guidance. Consult with GPs and specialist services to support comfort, symptom management, and appropriate clinical decision-making when residents approach the end of life. Medicines Management & Clinical Safety Lead safe systems for ordering, receiving, storing, administering, and destruction of medicines. Administer and monitor: Depot injections PRN medicines Monitor lithium and clozapine therapy, side effects, and therapeutic effectiveness, ensuring appropriate use of procyclidine. Oversee MARs and cream charts, ensuring medicines are administered and signed for correctly. Write and implement PRN protocols and cream protocols for new prescriptions. Complete random resident medication stock checks, escalating discrepancies and implementing corrective actions. Nutrition, Hydration & Dysphagia Ensure thickened fluids are prepared to the correct consistency. Monitor and audit fluid and nutrition records, ensuring no documentation gaps and act accordingly to weight loss or choke risks. Respond to Nourish warning alerts, ensuring care is delivered and late or missed tasks are followed up and closed down. Choke risk assessments are conducted. Make relevant referrals for specialist advice. Mental Capacity, DoLS & Legal Compliance Ensure Mental Capacity Assessments are completed, decision-specific, time-specific, and reviewed regularly. Ensure Best Interests decisions are lawful, proportionate, and clearly documented. Complete and monitor DoLS applications, including Form 1s and 2s, ensuring conditions are adhered to. Promote and evidence compliance with the Mental Capacity Act, Human Rights Act, and least restrictive practice. MDT Working & External Liaison Page 2 of 6 January 2026 Work in partnership with residents, carers, and professionals to deliver integrated care. Liaise effectively with: o Community Psychiatric Nurses o Social Workers GPs and specialist services o Commissioners and funding bodies Lead and participate in weekly GP rounds, ensuring actions are followed through. Coordinate and contribute to: o Funding reviews (612 monthly) o FNC reviews and placement reviews (612 monthly) Organise medical appointments and community outings, ensuring appropriate staffing and transport. Safeguarding, Risk & Governance Ensure the ongoing safety of residents by identifying, reporting, and actioning safeguarding concerns in line with policy and local authority procedures. Complete accident, incident, and safeguarding reports accurately and in a timely manner. Implement risk mitigation strategies and share learning with the staff team. Participate in clinical audit, governance, and quality improvement processes. Leadership, Workforce & Well-Led Responsibilities Lead the support team on shift, setting clear expectations and maintaining professional standards. Complete daily allocation sheets, ensuring: o Safe staffing levels o Appropriate skill mix o Staff breaks and task allocation. Mentor and support new starters and students, promoting a learning culture. Participate in staff supervisions and appraisals. Maintain own mandatory training and ensure staff compliance. Deputise for the Registered Manager in their absence, supporting service continuity, operational decision-making, and regulatory compliance. Documentation & Quality Assurance Ensure care plans, risk assessments, and records are accurate, person-centred, and up to date. Complete monthly reviews of all resident documentation to ensure true reflections of identified risks, needs and preferences. Maintain diary tasks and ensure key clinical and operational actions are completed. Provide clear, professional handovers and briefings. Ensure Nourish and Navimeds system is up to date with no gaps in records. Professional Responsibilities Maintain active NMC registration and adhere to the NMC Code. Engage in continuous professional development and reflective practice. Attend all mandatory training and required learning updates. Promote equality, diversity, dignity, and respect in all areas of work. Function as a positive role model, contributing to a calm, safe environment.
