Crynodeb
We are seeking to recruit an experienced Fixed Term Social worker to join the Hospital Discharge Team Based at the Royal Derby Hospital. Experience of working in a hospital setting is beneficial but not essential.
You will need to have the transferable skills to enable you to facilitate safe, sustainable transfers of care for people leaving hospital. To maintain system flow, this post will require you to work flexibly across the University Hospitals of Derby and Burton. The Post will also require you to work alongside the Care transfer hub, this may require you to work at Clay Cross hospital where the care transfer hub is based. The post is for a 5 in 7 working pattern but at the present time staff are expected to work on a Monday to Friday basis. If this is to change prior notice will be given in advance.
You will need to be able to manage change, also have the ability to maintain positive relationships with health partners and other agencies. You will need to be committed to working in a person-centred way with clients, their families and carers to maintain and maximise their independence, whilst observing the tight timescales under the Care Act and current guidance.
The successful candidate will be flexible and resilient, willing to adapt and continuously learn and cascade new skills and knowledge. You will be able to work as part of a multi-disciplinary team and share a clear sense of purpose and vision with resilience and determination to bring about change.
