Job Title: Care Coordinator (Care Operations) Reporting to: Head of Care (Nursing) Location: Horwich, Bolton with travel across the North West Hours: 35 hours per week (Monday Friday, 9:00am 5:00pm) On-Call: 1 weekend per month on a rolling rota Role Purpose The Care Coordinator is responsible for supporting the operational delivery of care services, ensuring all care packages are effectively staffed, coordinated and delivered to a high standard. Working closely with the Head of Care, the Care Coordinator ensures day-to-day operational readiness, staff coordination, training oversight and quality compliance across all active care packages. The role works collaboratively with Team Leaders and Senior Carers to ensure the wellbeing of staff, address operational challenges and respond to family needs in a timely and professional manner. Key Responsibilities Operational Delivery Support the Head of Care with planning and operational delivery of all care packages. Ensure care packages are staffed and scheduled appropriately within required timeframes. Monitor service delivery to ensure visits are completed safely and consistently. Referral and Onboarding Management Coordinate operational onboarding of new care packages including staff allocation, documentation and communication with families. Support the safe transition or offboarding of care packages where required. Rota and Workforce Coordination Develop and maintain staff rotas to ensure high coverage of care visits. Monitor staffing levels and collaborate with recruitment to address vacancies. Ensure weekend rotas are prepared and communicated in advance. Training Coordination Identify training needs within the Care Team in consultation with the Head of Care. Arrange and schedule required internal or external training. Confirm staff competency before they begin supporting families independently. Team Collaboration Work closely with Team Leaders and Senior Carers to support daily operations. Provide guidance and support where operational challenges arise. Escalate risks, family concerns or service issues to the Head of Care when required. Quality Assurance and Compliance Maintain accurate operational documentation including care records, training records and service notes. Ensure services operate in line with internal policies and regulatory expectations. Monitor operational risks and support improvements in service delivery. Family Engagement Act as an operational point of contact for families. Respond to concerns and support positive relationships with families receiving care. Reporting and Data Management Produce regular operational reports including rota coverage, training completion and service delivery performance. Maintain accurate digital records and systems. Travel and On-Site Support Travel across the North West region as required to support staff and care delivery. Conduct site visits to monitor quality and support the care team. On-Call Responsibilities Participate in the on-call rota one weekend per month. Respond to urgent operational issues and escalate concerns where necessary.
