Clinical Fellow - Same Day Emergency Care

The post holder will be clinically and educationally responsible to the Consultant/ GP working within the Same Day Emergency Care (SDEC) Unit, with collaborative responsibilities across the Acute Medical Unit (AMU) and Integrated Assessment Unit (IAU) to support patient flow and continuity of care within the acute medical pathway.
SDEC provides GP-led, same-day assessment, investigation, diagnosis and treatment for patients presenting with acute medical conditions who can be managed safely without overnight admission. The post holder will play a key role in delivering timely senior clinical decision-making, admission avoidance, and safe discharge planning within this model of care.
· In conjunction with the Specialty Doctor and Consultants, ensuring the effective organisation and clinical oversight of the SDEC service, including rapid assessment, diagnosis, investigation and treatment of patients referred for same-day emergency care.
· Ensuring that medical care delivered within SDEC aligns with agreed management plans, with clear documentation and timely clinical decision-making to support same-day discharge wherever clinically appropriate.
· Facilitating safe and timely discharge from SDEC, including completion of discharge documentation and clear communication with the General Practitioner to ensure continuity of care.
· Supporting patients flow across the acute medical pathway, including appropriate escalation or transfer to AMU or IAU where admission is clinically indicated.
· Working flexibly across SDEC, AMU and IAU as required to maintain safe service delivery, with SDEC remaining the primary base of practice.
· Under direct clinical supervision Consultant/ GP, supporting the training and supervision of junior doctors within SDEC and contributing to departmental teaching activities, including participation in Grand Rounds.
