About the Role
We are recruiting a dedicated staff team to support one individual with both mental health needs and a physical disability in his own home. This is a fully person-centred, values-based role, supporting him to live independently, maintain a meaningful daily routine, and engage safely with the community.
Key Responsibilities
Deliver high-quality, person-centred care in line with care and support plans.
Monitor mental and physical health, emotional well-being, and daily presentation.
Support all Activities of Daily Living (ADLs).
Promote independence and a structured, meaningful daily routine.
Support engagement in purposeful and enjoyable activities.
Assist with community access, appointments, and social opportunities.
Support with faith-based routines and religious practices.
Safely drive the individual’s vehicle.
Maintain accurate records in line with regulatory standards.
Uphold safeguarding responsibilities and professional boundaries.
Requirements
Registration or eligibility to register with Social Care Wales (SCW) - New staff can register once employed; support will be provided.
Enhanced DBS check.
Right to work in the UK.
Full UK driving licence.
Values-based approach and commitment to person-centred practice.
Good communication skills and ability to work as part of a consistent team.
Experience in mental health or physical disability support is preferred.
Ability to work flexible hours including days, nights, and sleep-ins.
Willingness to complete all mandatory training.
Drivers Requirement: Full UK driving license is essential – staff will be required to drive the individual’s own vehicle.
