Be responsible and accountable for the clinical pharmacy service provided to a sub-speciality.Participate in consultant and registrar led ward roundsoDrug history taking and medication reconciliation and review (including review of complex prescribed regimens)oPatient counselling and identification of patients who need additional help with their medications. Tailor the advice given to the needs of the individual, who may have learning difficulties, or other communication difficulties.Prioritisation of patients for early discharge and facilitation of discharge processEvaluation of the clinical appropriateness, safety and legality of prescriptions based upon the patients diseases, co-morbidities, and other medication and treatments taken, contacting the prescriber to make agreed changes as appropriate.Provide medicines information, and act as a resource on pharmaceutical care issues to other healthcare colleagues within specialty.Monitor effectiveness and adverse effects of medicinesDemonstrate professional accountability to patients.If post holder holds the relevant qualification to act as an Independent Prescriber. Make appropriate referrals where necessary; ensure the clear and effective communication of patients pharmaceutical needs to other pharmacists and health care professionals both within the trust and in other health care environments.Demonstrate appropriate clinical reasoning and judgement.Demonstrate awareness and commitment to the Trusts Clinical Governance Agenda. Responsible for the provision of Trainee pharmacists, foundation pharmacists and pharmacy student training programmesEnsure the placement incorporates both regional and national curriculum for Trainee and Foundation Pharmacist programmes by aiding in developing yearly training plansResponsible for assessing and signing off validations forms for pre-registration pharmacists and for facilitating, assessing and signing off Supervised Learning Events employed by the HEE LaSE (Health Education England, London and South East) Foundation pharmacist programmeTo support the identification, development and maintenance of clinical rotational training plans in liaison with the deputy chief pharmacist, lead specialist pharmacists and the Pharmacy Education and Development ManagerTo assist the Principal Pharmacist Education and Training in ensuring the quality of the training is maintained and monitored with the use of regular reviews and trainee feedback.To participate in the national recruitment of Pre-Registration Pharmacists.To assist in determining suitability of Foundation Pharmacists to undertake post graduate training programmesMonitoring trainee progress to ensure academic deadlines are met and liaising with relevant internal and external supervisors as requiredTo assist and support the Pharmacy Education and Development Manager in setting the strategy for providing the structure to support the delivery of the Foundation Pharmacist Programme in conjunction with educational supervisors and section heads.Identify and develop suitable practice and educational supervisors for training programmes in line with the department Education & Training Strategy (E&T) strategyParticipates in the annual review of the E&T and Quality Manual verified by HEE LaSE. To be an active member of the Pharmacy Education and Training team and Pharmacy Local Faculty Group (LFG).To deputise for the Pharmacy Education and Development Manager at Trust, local, regional or national level as required.To co-ordinate the undergraduate holiday placement scheme, working collaboratively with colleagues in the Surrey Heartlands STP and wider HEE LaSE region.To engage with other members of the pharmacy education and training team to ensure that learning and developmental needs of the department are being met and to engage other healthcare professionals as requiredTo act as a Practice Supervisor as required thus supervising and directing pharmacists and pharmacy technical staff in the delivery of high class pharmacy servicesResponsible for identifying the need for planned formal and informal teaching for members of Trust staff relating to any aspect of post holders areas of responsibility. E.g. core medical traineesTo maintain own continuing professional development (CPD) by keeping abreast of new trends and developments across the breadth of clinical pharmacy and pharmacy education & training.Assists in the delivery of the Trusts monthly Intravenous Additives Course for nursing staff teaching the principles of pharmaceutical aspects of safe intravenous administration of medicines.Assists in the delivery of training to the 5th year medical students and core medical traineesResponsible for organising and delivering the safe prescribing training and assessment for the foundation year 1 doctors as per HEE requirementsResponsible for organising morning/lunchtime educational sessions e.g. journal clubs, case studies, tutorials, training sessions, external courses, reflective practice, journal club and clinical supervisionDirectly train new pharmacists and pre-registration pharmacists on all aspects of clinical pharmacy services to own specialty area as directed by lead pharmacistsParticipate in and supervise new staff in the departments induction program.Develop strategies to meet the training needs of staff. Promote adherence to the Trust Formulary ensuring appropriate medicines are prescribed cost effectively and help to reduce wastage of medicines.Handle cash and card payments for prescription charges and OTC (over the counter) sales, if required.Responsible for the security of equipment and resources within the pharmacy department including books, computers, printers and subscriptions for web-based resources.Responsible for the safe and secure handling of medicinal products.Responsible for having a working knowledge of the pharmacy panic alarm system.Responsible for holding master keys for individual medicines lockersBe responsible for adhering to the pharmacy departments and RSCH Trusts security procedures for maintaining the security of the pharmacy department and the Trusts patients, staff and visitors.
