Crynodeb
The Pharmacy Technician will work as part of a multidisciplinary general practice team to support safe,effective and efficient medicines optimisation. Support safe and efficient repeat prescribing processes and ensure accurate processing of prescription requests with queries from different sources Assist with medication queries from patients, care homes,community pharmacies and prescribers, forwarding recommendations for alternatives Carry out medicines reconciliation for discharge letters,outpatient letters and new patient registrations Identify and resolve discrepancies and liaise closely with pharmacists and GP's Promote medicines synchronisation and encourage patients to adhere Support the implementation of local and national prescribing initiatives, including performing medication switches as directed by prescribers or pharmacists Maintain MHRA alerts and high risk drug monitoring records Support safe prescribing and monitoring of high risk medicines Contribute to medicines safety alerts implementation Provide training and guidance to non clinical staff as needed, to support prescription process within the practice Lead and contribute to audits and quality improvement related to safe and effective prescribing of medicines, using EMIS (Clinical system) Analyse prescribing data and share the learning with the wider team Manage medication queries and escalate appropriately Provide advice and information to patients on safe medicines use Support patient education and promote self care Maintain GphC registration with revalidation Adhere to practice policies,clinical governance frameworks and safeguarding procedures Develop and maintain effective relationships within the team and also with local stakeholders e.g ICB,community pharmacies and hospital pharmacy teams Engage in continuous professional development and attend clinical meetings within the practice.
