Crynodeb
The Clinical Pharmacist will: Work as part of a multidisciplinary team in a patient-facing role to assess and manage patients using their expertise in medicines. Undertake clinical medication reviews, particularly for patients with complex polypharmacy or long-term conditions. Support the care of patients in care homes and those with multiple comorbidities. Provide specialist advice on medicines to clinicians, patients and carers. Support safe and effective prescribing through medicines optimisation and quality improvement. Support and improve repeat prescribing systems within the practice. Respond to medication queries from patients and members of the practice team. Monitor patients on high-risk medicines and support appropriate therapeutic drug monitoring. Work with community and hospital pharmacy teams to support safe transitions of care. Maintain accurate clinical records in line with legislation and professional standards. Support clinical audit, education and quality improvement initiatives. Participate in clinical supervision and ongoing professional development.
