Crynodeb

Imperial College Healthcare NHS Trust’s strategy seeks to deliver our vision of ‘better health, for life’. It is rooted in a set of core values – to be kind, aspirational, collaborative and expert and is focused around three strategic goals:

- To help create a high quality integrated care system with the population of north west London.

- To develop a sustainable portfolio of outstanding services.

- To build learning, improvement and innovation into everything we do.

The work of the strategy, research, innovation & improvement directorate, which is led by our Executive Director Prof Bob Klaber, primarily focuses on the delivery of our third strategic goal ‘to build learning, improvement and innovation into everything we do’. The directorate includes sub-functions of improvement, innovation, strategy & partnerships, sustainability, population health & equity [to which these roles directly relate] and life sciences.

Whilst we aim to deliver high quality care in our 1.5 million patient contacts each year, we have a wider role in influencing the health of the north west London population. We are therefore seeking two passionate and engaging 'Population Health Senior Project Managers' to deliver an ambitious set of objectives to improve population health and health equity.
• To positively influence the health of north west London, we aim to embed a population health approach across all our services and find ways to better serve the communities outside of the hospital walls. Our population health approach focuses on health equity, prevention and our role as an anchor organisation.
• The two new senior project managers will join a developing population health team. In response to identified population health needs, the post holders will coordinate and deliver population health projects and improvement initiatives.
• Both roles will work closely with clinical, operational and community partners to design, implement and evaluate interventions that improve healthcare access, experience and outcomes for populations with the greatest health needs.
• One role will have a greater focus on delivering projects in collaboration with clinical services at the Trust. The second role will have a greater focus on engaging with local communities and working with voluntary and community sector organisations. There will be overlap in work areas, and both post holders will be expected to collaborate on health equity and prevention projects.
• These are two new fixed-term (24-month) roles. For both roles at least 50% in-person working is required at our main hospital sites. We are supportive of flexible working arrangements but cannot consider anything less than 0.8 whole time equivalent (WTE).

At Imperial College Healthcare you can achieve extraordinary things with extraordinary people, working with leading clinicians pushing boundaries in patient care.

Become part of a vibrant team living our values - expert, kind, collaborative and aspirational. You’ll get an experience like no other and will fast forward your career.

Benefits include career development, flexible working and wellbeing, staff recognition scheme. Make use of optional benefits including Cycle to Work, car lease schemes, season ticket loan or membership options for onsite leisure facilities.

We are committed to equal opportunities and improving the working lives of our staff and will consider applications to work flexibly, part time or job share. Please talk to us at interview.

The full job description provides an overview of the key tasks and responsibilities of the role and the person specification outlines the qualifications, skills, experience and knowledge required.

For both overviews please view the Job Description attachment with the job advert.
