Clinical Care and Therapeutic Interventions Lead the clinical management of care packages for individuals accessing the service. Coordinate and monitor person-centred healthcare using appropriate clinical decision-making. Deliver psychosocial interventions through individual and group therapy. Work collaboratively with the multidisciplinary team to assess needs, plan care, and respond to changes in service users health. Respect and promote service users customs, individuality, cultural background, values, sexuality, and spiritual beliefs. Provide therapeutic activities and emotional support while maintaining appropriate professional boundaries. Maintain supportive relationships with colleagues, professionals, and external agencies to enhance recovery outcomes. Manage a complex caseload as directed by the service. Communication and Documentation Record all contacts with service users accurately in case notes in accordance with SABP record-keeping policy. Maintain effective communication with service users, relatives, carers, visitors, and staff. Ensure communication aligns with organisational policies, procedures, and relevant legislation. Identify and implement strategies to overcome communication barriers. Seek informed consent for interventions and assessments in a meaningful and understandable way. Communicate in a manner appropriate to the individuals level of understanding, culture, and background. Quality and Service Development Contribute to the development, monitoring, and maintenance of service standards. Collect and collate data for audit, research, and service performance monitoring. Identify opportunities for service improvement and support implementation and evaluation of changes. Participate in complaint management processes by responding professionally and contributing to investigations. Promote shared decision-making with service users, relatives, and carers. Contribute ideas and suggestions for improving services and identify concerns within the workplace. Represent the team, service, and Trust positively. Risk Assessment and Safeguarding Understand and assess risks associated with the service user group and escalate concerns according to protocols. Conduct risk assessments and implement appropriate risk management strategies. Participate in the Patient Safety Incident Response Framework (PSIRF) by reporting and reviewing incidents to improve safety and quality. Recognise and respond to crisis situations promptly and responsibly, seeking assistance when necessary. Support service users families and external agencies when managing risk. Identify safeguarding concerns and follow relevant safeguarding policies for vulnerable adults and children. Ensure safe use of equipment and compliance with health and safety procedures. Provide health education related to risk factors affecting service users. Apply knowledge of relevant legislation, including the Mental Health Act, Mental Capacity Act, and Deprivation of Liberty Safeguards. Professional and Personal Development Engage in clinical supervision and appraisal to support professional development. Identify personal development needs and establish objectives with the appraiser. Attend all required statutory, mandatory, and essential training. Maintain evidence of competencies through a professional portfolio. Participate in the induction and orientation of new staff. Mentor students and provide a supportive learning environment. Management and Leadership Support recruitment, deployment, and management of staff in accordance with SABP policies. Assist the team leader in maintaining safe staffing and rostering in the clinical area. Ensure structures exist for appraisal, performance management, and staff development. Monitor and audit service standards regularly. Promote a healthy, inclusive, and diverse working environment. Ensure compliance with organisational policies, procedures, and financial governance requirements. Maintain professional confidentiality at all times.
