The States of Guernsey are looking for a Senior Team Leader to join the Social Care Team within Community Health and Social Care Services.
Working closely with the Social Care Team Coordinator, the Team Leader is responsible for the daily delivery of a high standard of assessment, planning, implementation and evaluation of care to Service Users within the Social Care Service.
This provision of assessed care will assist the Service Users to live as independently and safely, as possible, within their own homes.
