A Vacancy at University Hospitals Coventry and Warwickshire NHS Trust.


We are looking for a Specialist Doctor in geriatric medicine to join our team. This is a full time post within the Department of Care of the Elderly (COTE) at University Hospitals Coventry and Warwickshire
NHS Trust. The post will work in an integrated fashion with existing Consultant staff within the Department of Care of the Elderly (COTE) to deliver an expansion of services for older people and to support major new service development. The Specialist doctor will be expected to act autonomously and be responsible for patients under their care as an expert decision maker.

There are various opportunities to pursue sub-specialty interest and to develop new services including Community Geriatric services, Care for Elderly Major Trauma patients, Frailty Same Day Emergency Care (SDEC) service, Ambulatory and Hot clinics, Surgical liaison services, dementia services and, Oncogeriatric service. There are also opportunities to work together with other specialities to set up services such as joint Heart Failure services, movement disorder clinic and Fracture Liaison service.

As part of our commitment to patients and delivery of a world class service for all we have created the UHCW Improvement (UHCWi) System in partnership with the Virginia Mason Institute in Seattle; this involves a structured approach to removing waste and putting the patient first using a lean management system and methodologies. Our culture and ways of working reflect and embed the practices and methodologies of UHCWi. You are expected, where identified, to attend and complete relevant training and development opportunities to support this. This may include Lean for Leaders, Advanced Lean Training, and the Human Factors Programme, amongst others. Full attendance and completion of identified courses is considered essential and a pre-requisite for this post.

The post will work in an integrated fashion with existing Consultant staff within the Department of Care of the Elderly (COTE) to deliver an expansion of services for older people and to support major new service developments for older people at Hospital of St Cross, Rugby and University Hospital, Coventry. There are various opportunities to pursue sub-specialty interest and to develop new services including, Dementia service, Community Geriatric services, Care of Elderly Major Trauma patients, Frailty Same Day Emergency Care (SDEC), Ambulatory and hot clinics, surgical liaison services and Oncogeriatric service. There are also opportunities to work together with other specialities to set up services.

This is a full-time post within the Department of the Care of the Elderly at University Hospitals Coventry and Warwickshire NHS Trust. It is expected that appointees will have an in-patient bed base, contribute to out-patient services and be encouraged to develop a subspecialty interest. We welcome applications from those who would like to be considered for job sharing or less than full-time working. There are also additional PAs available depending on job planning.

You will be expected to contribute to the education and training of junior doctors and there will also be opportunities to contribute to teaching medical students from Warwick Medical School.

Department is currently pursuing the development of a major new strategy to develop and deliver Geriatric medicine services across Coventry and Rugby.

This strategy includes the following components:
• Acute frailty services – including front door frailty service, rapid access clinics, and ambulatory care, short stay assessment services.
• Provision of geriatric, medical input to Major trauma patients who are frail with multiple co morbidities.
• Surgical liaison Service – including pre op assessment to optimise medical issues for elective operation patients, peri operative liaison ward round for in-patients on surgical wards.
• Development of community services – including care home input and other support to primary care services e.g. development of virtual ward.
• Expansion of Falls and Bone Health services – including fracture liaison service, joint osteoporosis services, multidisciplinary falls clinic.

ON CALL COMMITTMENT
Current on call arrangements at University Hospital, Coventry and the Hospital of St Cross Rugby
The appointee is expected to participate in on call GIM take along with consultant colleagues.

Weekdays
All consultants in department are taking part in general medicine on call rota and new appointee is expected to join on call rota. Currently it consists of 1 in 16 based alongside consultants from other specialties including Endocrinology, Respiratory and Infectious Diseases. The on call consultant provides senior review of new patients admitted on Acute medical admission units between 5pm -9pm alongside the Acute Medical Consultant who is allocated to cover the unit in the evening. After 9pm, GIM consultant remains non-resident on call until morning and GIM consultant is not expected to do post take ward round in the morning. GIM on call consultant is given morning off in lieu equivalent to 1 PA after every on call.

On call GIM consultant in University hospital is also responsible to provide medical input for Rugby St Cross hospital once consultant is contacted by on call junior staff from Rugby St Cross.

Other specialty consultants who are not taking part in GIM rota such as cardiology, Gastroenterology, Renal run their own specialty on call rota to ensure patients are seen by right specialty in right time. All out of hours investigation facilities are available.

Weekend
Weekend GIM on call is currently is a 1 in 8 rota delivered by all the consultants in Care of the Elderly (COFE) alongside consultants from GIM rota .The GIM consultant on the weekend is expected to do one day – either Saturday or Sunday depending on the rota .

The duties involve resident on call duty from 8am - 3pm in University hospital Coventry. Starting with “shared post take ward round” with acute medical consultant in the morning on 34 bedded medical admission unit, followed by senior review of new GIM admissions at University Hospital,
Coventry. Second acute medical consultant will also cover the admission area from 1 pm to 8 pm.

On call GIM consultant has a Ward Round at Rugby St Cross hospital at 4pm to review new and sick patients only which last approximately an hour. After that, GIM consultant on call will be non-resident on call for both hospitals until the following morning.

Other aims and objectives
The post holders will be expected to deliver a first-class specialist-led service for older people inCoventry and Rugby. This will involve supervision and support of juniors so that services are provided in an effective and efficient manner, principles of risk management and clinical governance are adhered to and relevant targets are achieved.


The post holders will be an integral part of the Department of Care of the Elderly with line management from the Clinical Lead for the department.
The post holders will have opportunities to develop services and create new models of care to enhance the care of older people.
Leave cover will be provided by other Consultants within the Department of the Care of the Elderly and funding for this has been agreed.
