Crynodeb
Have clinical expertise in end-of-life care to support the professional development of Registered Nurses, Medical staff, Nursing Associates, Associate Practitioners, Health Care Assistants, AHPs and other members of the multi-disciplinary team as appropriate. Work with CHFTs end of life strategy to improve end of life and bereavement care within the Trust and across the local area. Competently manage any barriers to communication in order to effectively understand and meet individuals reported needs. Attend and develop the monthly Marigold bereavement caf. Organise, participate, and evaluate community engagements and events, including dying matters and grief awareness weeks. Support EOLC service improvement initiatives across the footprint of CHFT.
