Your Responsibilities The post holder is responsible for managing the teams undertaking of the assessment and discharge planning of patients within the defined clinical area; working under direction of their Complex Discharge Operational Lead. In addition, act as a case manager using their clinical knowledge and experience to support the day to day operational functions of the teams, completing their own assessments and interventions where indicated. You will be part of the senior leadership team offering teaching, supervision and operational direction, supporting the drive for quality and timely assessment to safely discharge patients. Each day can be quite diverse, supporting and responding to complex escalations which can offer variety and interest across the inpatient caseload. What were looking for You will be expected to be a confident communicator with excellent organisational skills. You will be required to liaise with family members, patients, and members of the Multi-Disciplinary Team both inside and outside of UHS on a regular basis and be competent with IT systems that facilitate this. You will need to demonstrate an ability to work in a busy and often pressurised environment, be able to provide highly specialised clinical discharge planning advice, both within the Trust and externally, to other team members; in support of wider health and social care professionals.
