Crynodeb
To provide direct access to patients seeking primary health care advice encompassing both face to face consultation and telephone access. Appointments will consist of face to face acute appointments, chronic disease face to face appointments and follow up and telephone reviews. The post holder is required to work within the policies and guidance set out by the practice and with due regard for the NMC code. Assess diagnose and plan treatments /interventions for pts presenting with an undifferentiated diagnosis, referring to community or secondary care services as appropriate. Assess and evaluate pts with complex needs and complete care planning documentation as required and to have comprehensive knowledge of chronic disease management ,processes and management. To work closely to NICE and local guidance for chronic disease management and prescribing. Proactively identify treat and manage those pts at risk of developing long term conditions. Perform medication reviews for therapeutic effectiveness appropriate to the pts needs and in accordance with EBP and national and practice policies and within own clinical competence. Take an active role in maintaining QOF register and responding to alerts in clinical practice. Liaison for QOF work should be directed towards lead GP for that particular clinical area. Support pts to adopt health promotion strategies and self-care.
