Crynodeb
To work as a registered nurse within the discharge lounge responsible to the Nurse coordinator /Senior sister caring for patients and their carers. Promotes and maintains positive and effective links of communication with patients, their carers and visitors to the Trust. Record the nursing care given, evaluating its outcome in terms of achievement of the objectives, modifying care documentation accordingly and become involved in audit / quality setting work of the department. Carry out holistic needs led assessments, with responsibility for team or onward referral as required, acting as the care manager for identified individual service users.
