Job Purpose The post holder will champion patient safety and good prescribing throughout the organization. They will identify appropriate clinical searches to identify patients who warrant a review or intervention related to prescribing. Amongst others, this will include follow up reviews or blood tests after medications have been newly started or periodic reviews for continuing medication. Having identified these searches, the post holder will run reports to identify relevant patients regularly. They will take steps to ensure that the appropriate review or intervention happens, where appropriate completing the review themselves. The post holder will work with the wider clinical team to identify appropriate searches and adjust these to reflect organizational and national priorities and evolving guidance. The post holder will also promote safe prescribing by ensuring that policies relating to prescribing, PGDs, PSDs and the way that these are implemented are consistent with regulator expectations and robust. Duties & Responsibilities 1. Leadership and Organisation Champion patient safety and safe prescribing throughout the organization. Take a proactive approach in identifying patients who require a review or blood test according to guidelines. Work with the wider clinical team to identify appropriate patient safety searches and adjust these to reflect organizational and national priorities and evolving guidance. Review and coordinate the appropriate action for all medication alerts. Ensure that policies relating to prescribing, PGDs, PSDs and the way that these are implemented are consistent with regulator expectations and robust. Assess compliance with prescribing guidance through clinical audit and use this as a tool to improve practice-wide prescribing. Report Significant events to help reduce avoidable medicines related harm and along with the GP Significant Event lead, take an overview of events related to prescribing in order to identify themes or areas for improvement. Attend daily clinical meetings to discuss complex patients and learning points. 2. Patient Care Be proactive in contacting these patients by text or phone or other means if appropriate to arrange or perform the required intervention or review. Complete outstanding patient medication reviews as required. Run and review regular reports to ensure patients starting specific key medication have had appropriate blood tests. Run and review regular reports to ensure all patients continuing specific key medications have had appropriate monitoring blood tests. Run and review regular reports to ensure that shared care agreements are in place and upheld where appropriate. Run and review regular reports to ensure that all patients on repeat medications who have not had recent medication review are reminded to book one. Run and review regular reports to review patients who are on controlled medications or potentially addictive medications. Run and review regular reports to identify patients with COPD/asthma who have been recently been prescribed medications for an infective exacerbation to ensure that they are reviewed, that their exacerbation has resolved and their medications are optimised. Work autonomously as an independent prescriber, while collaborating closely with GPs, nurses, physician associates, and other healthcare professionals to support high quality patient care and system efficiency. Demonstrate cost-effective, safe and appropriate prescribing, working towards ensuring that prescribing costs remain within the practices allocated budget and that prescribing follows local and national guidelines. Provide informal education and advice to practice staff, supporting consistent application of clinical guidelines and best practice in medicines use. Liaise with community pharmacies, secondary care, and other healthcare services to ensure safe transitions of care following hospital discharge or specialist review as required. This list is not exhaustive. The post holder may be also be requested to perform any other duties as reasonably required by the employer. Key Skills and Requirements Diploma in Clinical Pharmacy. Satisfactory CRB check. Effective and confident communicator at an individual level with patients, as well as in a larger group setting within the clinical team. Ability to work effectively within a team. Confident to challenge prescribing decisions of colleagues in a constructive way where appropriate. Leadership skills and experience of introducing and leading change. Ability to contribute to organizational strategy and planning. Confident using Clinical Software Systems, Microsoft Word, Excel and Powerpoint. Ability to manage a number of projects simultaneously at a high level, as well as performing clinical commitments. Resilient and able to cope under pressure. Able to cope with emotionally charged or difficult situations and/or patients. Self-motivated with ability to work autonomously. Reliable and punctual. Enthusiastic.
