Crynodeb
Working under the direct supervision of the GP’s and nursing team, strictly in accordance with specific practice guidelines and protocols, the healthcare assistant will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care. The post holder will: · Communicate effectively with all professionals involved with the patients care · Communicate effectively with patients and carers, recognising their needs for alternative methods of communication · To build professional rapport with patients · Provide and receive sensitive information · To report effectively to the team on patient’s progress and performance in relation to the patient. · To ensure up to date written and electronic records and activity data are maintained in accordance with professionals standards and are used for the purpose intended. · Participate in practice meetings as appropriate
