To provideadvice, support and direction to ensure holistic patient care for acutely ill and deteriorating patients across ELHT in conjunction with the multi disciplinary team over the 24 hour period. Work closely with the Intensive Care Nursing and Medical Teams to facilitate early and timely admission to critical care and safe discharge from critical care To provide the infrastructure that addresses the knowledge and skills deficits of ward teams in delivering acute care skills to patients. Support the Implementation of national guidelines or evidence based practice to improve patient care. Addresses clinical governance issues promptly. Provide leadership support for the Acute Care Team. The Advanced Clinical Practitioner will provide highly specialised care within the hospital setting, to those patients with chronic and acute conditions using advanced clinical knowledge that is evidence based The Advanced Clinical Practitioner will function independently or within a clinical team and willdemonstrate competence complementary to the MDT role in the delivery of health care. Provide expert advice and to ensure the needs of the patient are met by leading, challenging and changing practice within the clinical setting. To accept new and follow-up referrals To discharge patients from caseload as appropriate To undertake clinical audit and research Recognises and acts as an advocate for patients, carers, service and organisation. Lead change as required and ensure resolution of conflict. Undertake and lead quality improvement projects to support service development. Collaborate with the resuscitation team to develop and deliver education to improve cardiac arrest outcomes
