Job responsibilities The role will involve working with clinicians, managers, staff, professionals, partners, and other key stakeholders across health, social care, and multi-agencies to deliver a new model of care. Key responsibilities of the role will include: Undertake mapping of existing services and development of a detailed, robust case for change required as part of options and business case development. Leading on the development of new models of care, service plans/specifications and pathways in conjunction with health and care professionals, based on an agreed planning framework, national guidance and legislation, Powys Health and Care Strategy and Area Plan and local population needs and intelligence. Significant amount of engagement and communication messaging. Work with the Executive Leads, Senior Responsible Officers, Transformation Programme Managers, Operational and Clinical Leads and wider team members to provide leadership and expertise in technical planning to deliver transformational change in Powys. The post holder will be at the forefront of a dynamic and high-profile programme, making a significant contribution to all facets of health service technical planning.
