Crynodeb
Main Duties: To take the lead in the development of the in-reach liaison role demonstrating advanced levels of leadership and specialist clinical knowledge of the subject area. To contribute to the prevention of admission and facilitation of discharge of patients from Kingston Hospital within the Kingston Borough. To facilitate discharge from the front door of the hospital and provide rapid occupational therapy assessments at the patients home. And provide specialist care and advice to patients and/or their carers to influence better care and outcomes for patients. To work collaboratively with partner agencies optimising care in the right place at the right time for the patient. To act as a liaison between acute services and the community / local authority using expert knowledge of pathways and services. Provide clinical leadership for the development, implementation, and evaluation of frailty in community and acute clinical services, liaising closely with relevant colleagues and clinical experts in other organisations to establish excellent seamless pathways for patients.
