Crynodeb
To deliver Statutory Adult Social Care Services to people who meet the eligibility criteria of the Care Act 2014, and other relevant legislation by supporting the assessment of their needs and arranging support in meeting their outcomes and needs. To utilise services within the community optimising independence of residents, especially to enable them to remain within their own home or return to their own home following a hospital admission or other placement.
