We are working with a well-established, forward-thinking GP practice that is seeking an enthusiastic and committed Practice Nurse to join their friendly multidisciplinary team. This is an excellent opportunity for an experienced nurse looking to develop their career within a supportive primary care environment.

Key Responsibilities

• Provide high-quality nursing care across treatment room and chronic disease management clinics

  
  

• Undertake immunisations, cervical cytology, wound care, ear care, health checks and long-term condition reviews

  
  

• Support management of diabetes, COPD, asthma, hypertension and other long-term conditions within scope of competence

  
  

• Deliver health promotion, lifestyle advice and patient education

  
  

• Work autonomously while contributing to a wider multidisciplinary team

  
  

• Maintain accurate clinical documentation in line with practice protocols and professional standards

  
  

• Participate in safeguarding, infection control and clinical governance activity

  
  

• Contribute to quality improvement initiatives and the ongoing development of clinical pathways

  
  

  Requirements

  • Registered Nurse (NMC) with primary care experience

    
    

  • Confident in core Practice Nurse skills (immunisations, cytology, wound care, long-term condition monitoring)

    
    

  • Strong communication and patient-engagement abilities

    
    

  • Ability to work both independently and collaboratively within a GP practice environment

    
    

  • A positive, proactive approach with a commitment to delivering safe, high-quality care

    
    

  • Additional chronic disease qualifications or experience are highly desirable
