Crynodeb
We are looking to recruit a Clinical Nurse Specialist (CNS) to work within the Leeds Teaching Hospitals Trust (LTHT) Adult Palliative Care Team. The Palliative Care Team provides adult specialist palliative care services across all hospitals within LTHT. The CNS role includes working closely with referring medical teams to meet the specialist palliative care needs of patients and families. You will be responsible for managing a caseload of patients within LTHT. The role is primarily one of providing clinical advice, support and education. The post holder will be expected to work weekends and bank holidays. The jobholder is responsible for the management of a defined caseload. The jobholder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care. The jobholder will provide specialist nursing care, treatment and advice to a specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost-effective use of resources. The job holder will support the development of interdisciplinary and whole-service working. As a role model, they will promote the sharing of good practice and delivery of high-quality, effective and efficient care. The job holder will provide training and education to staff groups, patients and families/carers.
