Be professionally responsible for adhering to Trust Policies and Procedures. To undertake tasks in delivering patient care, rehabilitation or social care as directed by service professionals. To assist the nurses and other members of the multi-disciplinary team by carrying out delegated aspects of personal patient care. To carry out Physical Observation and escalate when need to To progress patient treatment, care planning and to observe any changes that affects the individual and accurately communicate these To work as a member of the multidisciplinary team and liaise with the team and other relevant agencies effectively to ensure co-ordinated approach to patient care and attend team meetings as required. Maintain timely and accurate records of work undertaken, including documentation in line with service standards. To prompt medication as directed by a registered health care professional.
