Key Duties and Responsibilities 1) Clinical responsibilities -Provide comprehensive primary medical care to registered patients, including assessment, diagnosis, treatment, prescribing and referral as appropriate. - Undertake a mix of face-to-face, telephone and online consultations in line with the agreed job plan. - Provide home visits when clinically appropriate and in line with our duty/visiting model. Sameday home visits are carried out by our urgent care team at the care centre, using our registrars and physician assistants (PA). Routine home visits are also managed by our PA team. GP home visits, while never completely avoidable, are rare. Applicants can expect around 34 visits per year, although there is no formal upper limit. - Manage acute presentations and contribute to same-day demand management according to practice and Urgent Treatment Centre (UCT) protocols. - Provide ongoing care for patients with long-term conditions, including medication reviews and evidence-based management. - Maintain accurate, contemporaneous clinical records and correspondence in line with NHS and GMC requirements. 2) Administrative and professional responsibilities -Complete clinical administration arising from consultations in a timely manner (e.g. results, referrals, letters, tasks), in line with the agreed job plan. - Participate in practice meetings, clinical meetings and multidisciplinary discussions as required. - Contribute to the practices clinical governance activity, including audits, significant event reviews and learning from incidents/complaints. - Support the training and development of the wider team where appropriate (e.g. medical students, GP trainees, ARRS staff), subject to experience and job plan. 3) Quality, safety and governance -Work within practice policies and procedures, including safeguarding, infection prevention and control, information governance and prescribing policies. - Contribute to maintaining standards required by the Care Quality Commission (CQC). 4) Teamworking and communication -Work collaboratively with GP colleagues, nursing team, practice management and the wider PCN multidisciplinary team. - Communicate effectively with patients, carers and colleagues, including shared decision-making and safety-netting. - Support a culture of mutual respect, continuous improvement and safe workload management. 5) Continuing Professional Development (CPD) and appraisal -Maintain GMC registration, revalidation and annual appraisal requirements. - Engage in CPD and reflective practice, with protected CPD time as set out in the job plan.
