The post holder will be hosted by Camden School Nursing Team but will work on a day-to-day basis with Camden Youth Justice Service, based at Crowndale, Camden. All work activity will be overseen and managed by the Service Manager for the Youth Justice Service but additional support and supervision will be available from School health colleagues.
The post holder will engage with children, young people, carers, and families, to ensure that the physical wellbeing and health needs of this client group are identified and responded to in a timely manner.

The aim of the post is to develop a universal physical and mental health offer for children known to Youth Justice Service (YJS) and children and young people (CYP), aged 10 – 25, at risk of offending, violence and exploitation. The aim is to strengthen relationships with key workers to promote improvements in the health and well-being of this client group. This will include health screening and signposting to relevant health services, liaison with relevant health colleagues and follow up/case management for children and young people with more complex and/or urgent health needs.

Role Specific Responsibilities


• Developing and delivering a health screening tool which will identify CYP’s physical health needs
• Triaging of cases to prioritise need and plan of care.
• Signposting to appropriate services, as and when required.
• Acting as an advocate for the CYP, where additional support is required
• Developing a database of all CYP engaged with YJS and at risk adolescent services to enable delivery of the health screening tool, capturing of outcomes/outputs, and inform commissioning and needs analysis
• Joint working with YJS colleagues including the provision of an overview of health needs (with CYP’s consent)
• Joint working with Children Looked After (CLA) service, including case discussion of CYP who are known to both CLA and YJS.
• Close working with Health specialists within the team collaborating where CYP have both physical and emotional health needs, when indicated.
• Developing relationships with all other key stakeholders to ensure children and young people access appropriate services in accordance with their physical health needs.
• Completing reports and engaging with relevant governance processes across health and the local authority, as and when required.
• Providing learning and development opportunities to YJS colleagues, to support better understanding of health needs and interventions.
• Providing the Health contribution to statutory processes (including safeguarding) as and when required.
Person Specification

Qualifications

Essential Criteria
•RGN/RN, RCN
•Evidence of relevant Continuous Professional Development

Desirable Criteria
• Mentorship qualification or Practice Assessor
•Nurse prescribing Qualification
•Specialist Community Public Health (SCPHN) qualification

Previous Experience

Essential Criteria
•Experience of working with vulnerable families and implementing evidence-based practice
•Experience of working in highly emotional/distressing circumstances
•Experience of participating in the completion of research/audits
•Experience of completing health assessments
•Evidence of multi-disciplinary integrated team working
•Ability to work flexibly and respond to changing demands both individually and within a team.

•Knowledge of Safeguarding Children Issues
•Experience in supporting the development of other practitioners

Desirable Criteria
•Experience of working with the Youth Justice Service

Skills and Knowledge

Essential Criteria
•Knowledge of clinical governance, patient safety, and risk management principles
•Ability to interpret and present complex information clearly to varied audiences
•Knowledge of safeguarding children and vulnerable adults
•Demonstrate an understanding of the health needs of this cohort of young people
•Demonstrated an understanding of the SEND agenda
•Knowledge of health issues and relating to, and impact of Adverse Childhood Experiences
•Understanding of NMC Code of Conduct and scope of professional practice

Communication:
Able to communicate with the multi-disciplinary team, present cases, take note of actions, update others in the team. Speaking face to face, by telephone and writing letters and reports.
Maintaining accurate, timely, concise and legible clinical records both electronic and handwritten which adhere to the CNWL policies
Able to talk to people about sensitive issues in a supportive manner but also challenge others assertively where necessary


IT Skills:
Good working knowledge of data systems (e.g. SystmOne, or equivalent)
Able to maintain electronic patient records
Use Microsoft Word for letter and report writing
