Summary

To facilitate renal replacement therapy on paediatric patients with end-stage kidney disease at home and in hospital by providing a training programme, continual support and monitoring. To work closely and cross cover all renal replacement modalities, including peritoneal dialysis, incentre haemodialysis and home haemodialysis.

The post holder will be expected to provide clinical expertise and knowledge, demonstrating a sound understanding of the issues related to the identification, assessment, treatment and management of the client group. S/he will monitor and improve standards of care through, supervision of practice, clinical audit, evidence based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership. S/he will actively contribute to the clinical governance processes within the service. This role requires flexibility in order to meet the needs of the entire dialysis service.

As a dialysis nurse specialist you will be expected to work across the whole dialysis service providing a high standard of care.

Competencies will be achieved in peritoneal dialysis home haemodialysis and incentre haemodialysis.

Day to day work will vary according to the needs of the dialysis service, this requires flexibility and adaptability and excellent communication skills.

Responsibilities include:

Delivering high standards of care for all dialysis patients

Training patients and their families in all modalities of dialysis

Conducting home assessments and home visits including supporting first dialysis sessions at home

Reviewing data and optimising dialysis regimes

Preparing for reviews and clinics

Participating in clinics with MDT and leading on nurse led clinics

Participation in local and national study days

Training ward staff

Autonomous working within a nurse led environment

Community networking and collaborative working
