Key Responsibilities1. Patient-Facing Clinical Work Conduct structured medication reviews (SMRs) Manage long-term conditions (e.g. hypertension, diabetes, asthma, COPD) Independent prescribing within scope of competence Manage acute minor illnesses (if qualified) Undertake polypharmacy reviews, especially in elderly or frail patients Deprescribing where appropriate 2. Medicines Optimisation & Safety Reconcile medicines following hospital discharge Review high-risk medications (e.g. DOACs, methotrexate, lithium) Monitor blood tests and therapeutic drug levels Implement local and national prescribing guidance (e.g. National Institute for Health and Care Excellence (NICE)) Lead audits to improve prescribing safety 3. Repeat Prescribing Management Authorise repeat prescriptions Manage medication queries from patients and community pharmacies Improve repeat prescribing systems and processes Ensure cost-effective prescribing in line with formulary 4. Clinical Governance & Quality Improvement Conduct prescribing audits Reduce medication errors Support CQC compliance (Care Quality Commission) Develop and update practice prescribing policies 5. Multidisciplinary Team (MDT) Working Work closely with GPs, nurses, paramedics, and social prescribers Provide medicines advice to the practice team Support care homes with medication reviews Liaise with community pharmacies and secondary care 6. Patient Education & Support Counsel patients on medication use Support adherence Provide lifestyle and self-care advice Manage shared decision-making discussions
